The self-assessment is based on the ASSIST* questionnaire. It asks questions about your use of meth, as well as other drugs. It is definitely not a diagnostic test, nor is it intended to replace a real assessment from a health professional, but it's a good place to start. It will take about 5 minutes, and even though it seems like some questions are repeated, it's really worth taking the time to finish it. Your answers are totally anonymous so this is a great opportunity to be really honest with yourself.
